Full fees are charged for missed appointments unless a cancellation was made 48 business hours prior to the appointment.

If you need a refill between visits, please ask your pharmacy to fax the request to 206-385-3281.

Your information is confidential with the following exceptions:

1) when you have signed a release of information 

2) if safety is a concern

3) court subpoena/orders

4) I am obligated to report abuse cases
